Call the Registration Department at (719) 543-8715 before you make a medical appointment. You may be asked to provide your name, address, phone number, insurance information, and other information. Arrive 15 minutes early for your first appointment to fill out a short registration form. Please bring your Social Security card and private insurance/Medicaid/Medicare card(s).

I don’t have health insurance.
If you do not have health insurance, you must make an appointment to register as a patient by calling the Registration Department at (719) 543-8715 before you make a medical appointment. PCHC offers an informational booklet called “Sliding Fee Program Guide,” which explains the details of applying for our sliding fee program.
