History

Pueblo Community Health Center began providing primary health care services to Pueblo’s underserved population in 1983. Originally housed on the grounds of the Colorado State Hospital, Pueblo Community Health Center moved to St. Mary-Corwin Hospital in 1986. In 1987, Pueblo Community Health Center purchased a building at 110 East Routt Avenue, which is currently a medical health center. The 310 Colorado Avenue building was purchased and renovated in 1991 and now houses the patient pharmacy and some administrative offices.

In 2000, Pueblo Community Health Center acquired and renovated the 2030 Lake Avenue site to further expand our capacity to provide dental services. Primary health care services are also provided through a satellite facility at 328 Avondale Boulevard in the rural community of Avondale; Homeless patients may receive medical care at Posada, a local homeless services center centrally located in downtown Pueblo at 1008 North Grand Avenue.

In October 2002, Pueblo Community Health Center opened Park Hill Medical Health Center at 1302 East Fifth Street in the heart of Pueblo’s most underserved neighborhood. As demand for services continues to grow, Pueblo Community Health Center will soon open the lower level of the Park Hill Site.

Pueblo Community Health Center served over 17,000 patients with more than 88,000 visits in 2004.

Recent Accomplishments

  • We served 18,229 patients, up 1,000 (+5.8%) over 2005
  • We provided 83,583 medical visits, up 4.4% over 2005
  • We provided 7,137 dental visits to 3,155 patients, up 3% over 2005
  • We provided 2,310 mental health visits
  • We delivered 509 babies and enrolled 637 new patients into prenatal care, both new records
  • We provided 93,030 total visits, up 4% over 2005
  • Our patient satisfaction scores remained above 95% in almost every area and 99% of our patients would recommend PCHC to others
  • We completed our 11th consecutive year in the black
