Becoming a Patient

We’re glad you are interested in joining Pueblo Community Health Center! Our services top quality, whether you need a routine physical exam, prenatal care, one-on-one help managing your diabetes or depression, or any other service we provide. We are here to help YOU!

Before you are seen by one of our providers, we will need some basic information. Please click on the category that best suits your needs:

Call the Registration Department at (719) 543-8715 before you make a medical appointment. You may be asked to provide your name, address, phone number, insurance information, and other information. Arrive 15 minutes early for your first appointment to fill out a short registration form. Please bring your Social Security card and private insurance/Medicaid/Medicare card(s).

I don’t have health insurance.

If you do not have health insurance, you must make an appointment to register as a patient by calling the Registration Department at (719) 543-8715 before you make a medical appointment. PCHC offers an informational booklet called “Sliding Fee Program Guide,” which explains the details of applying for our sliding fee program. You must provide proof of income to apply for the program.

Patient Medical Records

We respect your right to confidentiality. Pueblo Community Health Center will release information regarding your health care only with written permission from you. If you or an authorized representative (other than your doctor) requests a copy of your medical records, we will copy and mail records to another doctor at no cost to you. We will charge you a copying fee of $14.00 for the first 10 or fewer pages, 50¢ per page for pages 11-40, and 33¢ per page over 40 pages if we release the records to you. It will take approximately 20 working days to process a request for a copy of your medical records. Note: The copying fee is subject to change without notice. You will be charged any postage fees for mailing the records. Records will be forwarded when full payment for copy charges and postage has been received. To confirm rates or if you have other questions, please call 719-543-8711 and ask for the Medical Records department.

Patient Accounts

Questions about your bill should be directed to our Billing Department at (719) 543-8716. Patient Account Representatives are available from 9:00am – 5:00pm, Monday through Friday. You may also contact us by Email: Billing@pueblochc.org

Insurance

As a courtesy, Pueblo Community Health Center will gladly submit your claim to your insurance company for you. The patient’s insurance information and card must be submitted at the time of service.

You Have the Right...

  • to expect quality care and reasonable access to primary heatlh care.
  • to be treated with respect and dignity.
  • to the assurance of confidentiality of your medical information.
  • to receive information about Pueblo Community Health Center's services, care providers, and organizational policies.
  • to choose primary health care providers, including the right to refuse care from specific providers.
  • to have your pain assessed.
  • to participate in all decisions involving your care and treatment, including the right to refuse any drug, procedure, test or treatment.
  • to appoint someone to help you with care decisions if you cannot make those decisions for yourself.
  • to receive information about advance directives, as well as receive help in making an advance directive.
  • to know the immediate and long-term financial implications of treatment alternatives, insofar as they are known.
  • to know if the facility is participating in teaching programs, research and/or experimental programs.
  • to have access to your medical records in accordance wiht Federal and State laws.
  • to voice complaints about the care provided or about the organization.
