Dental Services
You must be a medical patient at Pueblo Community Health Center to receive dental care. Our dentists provide basic dental services for pediatric and adolescent patients. Limited dental services are provided for adults. Please call our Registration Department at (719) 543-8715 for more information.

Nurse-Family Partnership

With your first pregnancy...

You may have mixed feelings and be unsure about the future. The Nurse-Family Partnership program can help. Our nurses are here to answer your questions and listen to your concerns.

This is a voluntary program and the services are provided to qualified Pueblo County residents at no cost to you.

During Pregnancy...
Your personal Registered Nurse Home Visitor will meet with you and help you:

  • find answers to your questions about being pregnant
  • make healthy choices about your diet and pregnancy
  • prepare you for labor and delivery

After you deliver your baby...
Your Nurse Home Visitor will continue to visit until your child is two-years-old. She will:

  • give you information about breast- and bottle-feeding
  • provide helpful hints to make parenting easier
  • teach you about normal growth and development
  • inform you about child health and safety issues
  • help you plan for the future

Family and Community Connections...
We encourage early and continuous prenatal care, personal development, and the involvement of fathers, grandparents and other supporting persons in parenting.

We also make referral for health care, child care, mental health care, job training, and other appropriate services available in Pueblo County.

Migrant/Seasonsal Farmworker Services
Pueblo Community Health Center is proud to offer special services and rates for Pueblo County’s agricultural community. During annual peak growing seasons, we extend our hours of operation and visit migrant/seasonal farmworkers in the fields of Eastern Pueblo County. We encourage preventive and routine physical exams, as well as appropriate immunizations. These special residents are especially susceptible to work-related infections and injuries, so Pueblo Community Health Center provides the education and direct services needed to protect the agricultural workers.

Homeless Services
A homeless person is an individual without permanent housing who may live on the streets; stay in a shelter, transitional housing, mission, single room occupancy facilities, abandoned building or vehicle; or any other unstable or non-permanent situation.

An individual may also be considered homeless if they are unable to maintain their housing situation and are forced to stay with a series of friends and/or extended family members. Homeless individuals who are to be released from a prison or a hospital may be considered homeless if they do not have a stable housing situation to which they can return.

Pueblo Community Health Center is proud to offer health care services to the homeless people of Pueblo . Our Health Care for the Homeless program is housed at Posada, Pueblo ’s homeless services site.

HIV/AIDS Services
The Ryan White Title III, Early Intervention Services is for people infected with or at high risk of acquiring HIV/AIDS. Services are available at a reduced cost for low-income or uninsured patients.

Pueblo Community Health Center provides outpatient early intervention and primary medical care services for people with HIV/AIDS in Pueblo, Alamosa, Baca, Bent, Crowley, Custer, Fremont, Huerfano, Las Animas, Otero, and Prowers Counties.

Program Services include:

  • HIV counseling, testing and referral services
  • Medical evaluation and clinical care
  • Pharmaceutical care
  • Oral health care
  • Drug therapy adherence counseling
  • Nutritional counseling
  • Outpatient mental health care
  • Outpatient substance abuse counseling
  • Referrals and coordination with specialty care
  • Coordination with Social Services

Patient Pharmacy

Pueblo Community Health Center ’s Patient Pharmacy accepts the PCHC Member card, as well as many private and public insurance plans. Please check with your provider or the Patient Pharmacy at (719) 543-4545 to verify that your plan is accepted. Only Pueblo Community Health Center patients may use this pharmacy.

Co-Payments
Insurance Plans
Pueblo Community Health Center requires patients to pay co-payments at the time you receive your medicine. There may be a co-payment for each prescription filled. Sometimes the actual cost of your drugs will be less than your co-payment. In this case, you will only be charged the actual cost.

PCHC Member Card Plan
Pueblo Community Health Center requires patients to pay co-payments at the time you receive your medicine. There is a co-payment for each prescription you have filled. Your co-payment amount is indicated on your PCHC Member card.

Formularies
A formulary is a list of drugs for which your health plan provides benefits. You are responsible for the entire cost of your medicine when a prescription drug is not on your plan's formulary.

Insurance Plans
Consult your provider or call the number on your insurance card to obtain information about what is covered on your plan’s formulary.

PCHC Member Card Plan
There is a formulary for the PCHC Member card plan and it is designed to meet the medication needs of our patients. There may be some medicines that are prescribed which are not on the formulary and you will be responsible for the entire cost. Your provider will work with you to provide medicine that is covered by the formulary. Controlled substances are not on the formulary.

After Hours and Holiday Service
If you need prescription service while the Patient Pharmacy is closed, please call (719) 543-8711 and you will be connected with our answering service. If needed, the doctor will call your prescription into another pharmacy; however, it will not be covered by the PCHC Member card plan. If you like, you could obtain enough medicine to last you until the next day the Pueblo Community Health Center Patient Pharmacy is open; you do not need to have the entire prescription filled.

Prescriptions from the Emergency Room or Non-Pueblo Community Health Center Doctor
Only prescriptions written by a Pueblo Community Health Center doctor are accepted at the Patient Pharmacy.
Prescriptions written by the emergency room doctor or other non-Pueblo Community Health Center doctor cannot be filled at our Patient Pharmacy. Please call your Pueblo Community Health Center provider at (719) 543-8711 to discuss options for getting your medicine at our Patient Pharmacy. In some cases, you may have to go to a pharmacy where the PCHC Member card is not accepted.

Refills
Whether or not you have refills remaining on your current prescription, please contact the Patient Pharmacy at (719) 543-4545 at least three days before you need the prescription.

Drug Subsidy Program
Certain pharmaceutical companies allow Pueblo Community Health Center to apply for discounted prescriptions on your behalf. The prescriptions are usually for health maintenance needs.

Eligible Patients
1) Current Pueblo Community Health Center Member card patients in sliding fee categories of N, A, B, C, D or E may be eligible to apply for the Drug Subsidy Program.

2) Your prescription must. . .
. . .be ordered by a Pueblo Community Health Center doctor.
. . .be a current medicine you are using.
. . .be available from a participating pharmaceutical company’s patient ssistance program.

3) You must give Pueblo Community Health Center permission to apply for Drug Subsidy Program services on your behalf.

Service Fees
There is a minimal service fee for prescriptions received through this program.

Other Costs
Depending on the medicine, you may be required to pay your PCHC Member card co-payment until Pueblo Community Health Center receives the medicine from the pharmaceutical company. This means you will pay your co-payment for at least your first prescription. The medicine for this program is usually received before you need a refill. Pueblo Community Health Center orders your medicine promptly, but it is up to the pharmaceutical company to determine how quickly the medicine is delivered.

Refills
The Patient Pharmacy has a system for reordering your medicine from the manufacturer. We will contact you when your reordered medicine arrives. In the event that your medicine runs out before the manufacturer sends a refill, you will need to obtain your medicines on your PCHC Member card and pay the co-payment.

Non-Compliance
Pueblo Community Health Center reserves the right to discontinue your participation in the Drug Subsidy Program if you lose eligibility as defined above or do not comply with the program.

Women's Cancer Case Management Program
Thanks to the perseverance of Pueblo Community Health Center 's thorough case management services through the Colorado Springs Affiliate of the Susan G. Komen Foundation, as well as the Colorado Women's Cancer Control Initiative, more than 1,800 women received cancer screenings in 2004! We were able to detect many early stage cancer patients and arrange for treatment.

Our comprehensive case management program educates the community about the importance of preventive services for breast health and care by encouraging regular exams/screenings and other preventive health care measures. Case management staff members closely monitor Pueblo Community Health Center ’s female patients and provide personalized, comprehensive case management services for breast cancer screenings and follow-up care.
