The vision of PCHC is to be the primary health care provider of choice in Pueblo County by offering top quality care through accessibility, leadership and financial independence.

In 2018, PCHC proudly accomplished the following:

  • Served 25,878 patients
  • Provided 104,919 medical visits
  • Provided 11,707 dental visits
  • Provided 11,643 behavioral health visits
  • Continued to register patients on a same day basis
  • Delivered 505 babies. This is one out of every four babies born in Pueblo County
  • Of patients with high blood pressure, 76.3% are controlled
Please call (719) 543-8711 before coming to a clinic so we can determine how best to safely meet your needs. All patients/visitors are required to wear a mask while in any PCHC facility. The PCHC dental clinic is open and treating emergencies and providing other limited services - call 564-4823 for more information. PCHC is referring patients who need a COVID-19 test to the Colorado State Fairgrounds testing site (open Monday through Friday, 10 a.m. to 7 p.m. and every second and fourth Saturday of the month from 10 a.m. to 7 p.m.)

AAAHC Notice of Accreditation Survey

Pueblo Community Health Center has voluntarily requested this accreditation survey as a means of having a third-party review of the entire organization to build upon strengths or identify opportunities to improve its delivery of safe, high-quality health care. PCHC’s AAAHC (Accreditation Association for Ambulatory Health Care, Inc.) survey dates are set for October 26-27, 2020.
