Pueblo Community Health Center strives to provide the highest quality care possible to our patients. We are happy to report that three out of four PCHC patients see their primary care provider every single year and that our patients exceed the Healthy People 2020 goals for hypertension control, 1st trimester of entry to prenatal care, and HIV linkage to care. We are accredited by the Accreditation Association for Ambulatory Health Care, and we receive support from Pueblo County United Way and Health Resources and Services Administration in order to provide our patients with the best primary care available in Pueblo County.
The AAAHC Mission: To encourage and assist ambulatory health care organizations to provide the highest achievable level of care for recipients in the most efficient and economically sound manner. AAAHC accomplishes this by the operation of a peer-based assessment, consultation, education and accreditation program.
