PCHC Wins Gold Quality Award for Fourth Year in a Row
(Pueblo, CO, Sept. 23) – For the fourth year in a row, Pueblo Community Health Center (PCHC) has been named a Health Center Quality Leader at the top level of “Gold” by the Health Resources and Service Administration (HRSA) of the federal government. This award recognizes PCHC placing in the top 10% of all health centers for clinical quality measures nationally. Only three of 19 community health centers in Colorado achieved this award.
As Federally Qualified Health Center (FQHC), PCHC provides comprehensive primary and preventive care in a patient centered medical home setting. This team-based approach is led by a health care provider and includes case management, nursing, care coordination, and targeted disease management support which may also include a health educator and pharmacist. PCHC also provides OB, behavioral health, and dental services. According to the National Association of Community Health Centers (NACHC), not only do community health centers perform just as well or better on quality measures, but they also provide care at lower costs.

The quality data and awards are important, but we do not forget that it is a privilege and an honor to serve each patient who entrusts us with their health care. Each patient is a friend, a neighbor, a family member, or other valuable part of our community,” stated Lynne Evetts, Director of Quality at PCHC.
PCHC also received an award for Advancing HIT (Health Information Technology) for Quality. PCHC has a robust electronic health record system, exchanges clinical information online with key providers, and offers technology to patients such as telehealth services, and a patient portal. Patient social risk factor data is also collected to help better serve patients.
