Pueblo Community Health Center Opens New Wellness Center

The newest Pueblo Community Health Center School-Based Wellness Center (SBWC) celebrated its grand opening on Tuesday, October 8, 2019 at Chávez-Huerta K-12 Preparatory Academy (CHPA). The ribbon cutting ceremony was hosted by the Latino Chamber of Commerce at 4:00 p.m. at the Wellness Center, which is located at 2500 West 18th Street in the grey building, #11 (next to the school gymnasium).

The CHPA School-Based Wellness Center was recently remodeled and furnished, and features a reception area, behavioral health evaluation room, break room, lab, and two newly equipped exam rooms.

Pueblo Community Health Center (PCHC) offers additional School-Based Wellness Centers in the following schools: Central High School, East High School, Centennial High School, Risley International Academy of Innovation, and Pueblo County High School. In addition, there is a clinic on the campus of Pueblo Community College. Primary medical and behavioral health care services (including sports physicals) are offered at all locations and most insurance plans are accepted.

The clinics are staffed by Nurse Practitioners, Physician Assistants, RN’s, MA’s and receptionists. Any student enrolled in the district (ages 3 and up) are eligible to receive services at the SBWC’s. A signed consent form must be completed and on file. CHPA is presently a closed campus (only CHPA students can be seen at this clinic), but all other SBWC clinics are open to all students in the district. School staff can also be seen with a signed consent.
