Pueblo Community Health Center Wins National Quality Awards

(Pueblo, CO, Nov 28, 2023) -- Pueblo Community Health Center (PCHC) is pleased to announce that they have been named a Health Center Quality Leader awardee by the Health Resources and Service Administration (HRSA), a division within the U.S. Department of Health and Human Services. This award recognizes the highest performing health centers nationwide. As a Health Center Quality Leader, PCHC achieved the best overall clinical performance among all health centers nationally, placing in the top 10% for clinical quality measures. Only four of 20 community health centers in Colorado achieved this award.

A Federally Qualified Health Center (FQHC), PCHC provides comprehensive primary and preventive care in a patient centered medical home setting. This team-based approach is led by a health care provider and includes case management, nursing, care coordination, and targeted disease management support which may also include a health educator and pharmacist. PCHC also provides behavioral health and dental services.

“PCHC is quality driven. The pursuit of top-quality patient care is central to how each staff member supports our mission, which is to provide primary care to those in need. We strive to provide that care in a manner consistent with our core values, to every patient we see,” stated Lynne Evetts, Director of Quality at PCHC.

Compared to peer community health centers nationally, PCHC was ranked based on quality of care and health outcomes and disparities. Quality of care measures long-term health outcomes and timely and routine preventive care such as cancer screenings and diabetic management. Health outcomes and disparities measures the impact of clinical intervention to project long-term health outcomes. Medicaid clients who use FQHCs are less likely to have an emergency room visit, an inpatient hospitalization, or preventable hospital admission. According to the National Association of Community Health Centers (NACHC), not only do health centers perform just as well or better on quality measures, but they also provide care at lower costs.

PCHC also received awards for being Advancing HIT (Health Information Technology) for Quality, Access Enhancer, Addressing Social Risk Factors for Health, and Health Disparities Reducer.
