School-Based Wellness Centers

In partnership with Parkview Medical Center, Pueblo City Schools, Pueblo School District 70, and Health Solutions, Pueblo Community Health Center is responsible for School-Based Wellness Centers at Centennial High School, Central High School, Chávez Huerta K-12 Preparatory Academy, East High School, Pueblo County High School, and Risley International Academy of Innovation.

School-Based Wellness Centers provide comprehensive primary and behavioral health care comparable to services received at a family practice physician's office, behavioral health counseling office or clinic. PCHC's health care services and Health Solution's behavioral health services will be provided according to standards of care accepted by the medical community to assure the highest quality of care.

Please call (719) 543-8711 before coming to a clinic so we can determine how best to safely meet your needs. All patients/visitors are required to wear a mask while in any PCHC facility. The PCHC dental clinic is open and treating emergencies and providing other limited services - call 564-4823 for more information. PCHC is referring patients who need a COVID-19 test to the Colorado State Fairgrounds testing site (open Monday through Friday, 10 a.m. to 7 p.m. and every second and fourth Saturday of the month from 10 a.m. to 7 p.m.)

AAAHC Notice of Accreditation Survey

Pueblo Community Health Center has voluntarily requested this accreditation survey as a means of having a third-party review of the entire organization to build upon strengths or identify opportunities to improve its delivery of safe, high-quality health care. PCHC’s AAAHC (Accreditation Association for Ambulatory Health Care, Inc.) survey dates are set for October 26-27, 2020.
