Pueblo Community Health Center (PCHC) offers a Patient-Centered Medical Home for all who seek care. Our practice model emphasizes a personal relationship with your Primary Care Provider (PCP). Your PCP leads and coordinates services with the help of a team of professionals. Our goal is simple: better care for you. We place importance on comprehensive, continuous attention to you, for your improved health outcomes.

Extensive Care Options

Medical Services

Pueblo Community Health Center’s Board Certified family practice physicians, physician assistants, nurse practitioners, and nurse midwives offer the following services for you:

•  Preventive care for children and adults
•  Routine check-ups
•  Immunizations
•  Family planning and birth control
•  Cancer screenings
•  Care for acute and chronic illnesses
•  Treatment for minor injuries
•  Minor surgery
•  Pharmacy
•  Laboratory

Dental Services

Our dentists provide basic dental services for adult, adolescent, and pediatric patients. You must be a medical patient at Pueblo Community Health Center to receive dental care. Please call our Registration department at (719) 543-8711 for more information.

Prenatal Care/OB-GYN

A full range of comprehensive health services is available to women before, during and after pregnancy.

These services include the following:

  • Free pregnancy tests
  • Prenatal care and delivery
  • Postpartum care
  • Breastfeeding education support and assistance
  • Newborn care
  • Ultrasound, fetal monitoring, and complimentary 3D imaging
  • Prematurity prevention sessions
  • Childbirth classes
  • Birth control
  • Home visits
  • Care provided by Certified Nurse Midwives or Physicians
  • Supportive partnerships with local support programs

Behavioral Health Services

Licensed behavioral health therapists help our patients experiencing issues such as depression, anxiety, grief, and trauma.

Available behavioral health services include the following:

  • Brief solution-focused therapy for individuals, families, and couples
  • Psychotherapy
  • Acudetox
  • Group therapy, including therapeutic and educational modalities
  • Crisis intervention
  • Access to psychiatric medication consultation

Comprehensive Care

Pueblo Community Health Center offers integrated care to its patients. This means coordinated medical, dental, and behavioral health care for all, with on-site podiatry (foot care), care coordination, pharmacies, and laboratories.

PCHC treats the “whole person” and all services required for you to achieve optimal health.

Behavioral Health Services

Pueblo Community Health Center employs licensed behavioral health providers who are passionate about assisting patients in achieving their goals for mental and emotional well-being. Behavioral health providers can offer solution-focused therapy for individuals, families, and couples, as well as psychotherapy, group therapy, Acudetox, crisis intervention, and access to psychiatric consultations. Lynne Evetts Nominated for 2021 Nightingale Award

Pueblo Community Health Center (PCHC) is pleased to announce that Lynne Evetts, RN, FNP, was nominated for the 2021 Nightingale Award. Presented by the Colorado Nurses Foundation, this award is Colorado’s highest nursing honor, and it recognizes nurses who advance their profession and positively impact their community in exceptional ways.

Low-Cost Sports Physicals offered in Pueblo School Districts 60 and 70

Pueblo Community Health Center (PCHC) is again offering affordable sports physicals for athletes in both middle and high school enrolled in either Pueblo School Districts 60 or 70. Students DO NOT need to be seen at the school that they attend.

East Side Clinic Featured in Latino Chamber April Virtual Net-Walk

On Wednesday, April 20, 2021, The Latino Chamber of Commerce hosted a virtual Net-Walk in the East side of Pueblo. They featured businesses and developments currently taking place on the East side, including the construction of the Pueblo Community Health Center East Side Clinic.
